I consent to my youth being a member of the EBC Youth community (through onsite activities or online engagement) as an extensiton of Edwardstown Baptist Church (EBC).

I consent to my/my youth's details being added to the weekly mailing list (SMS).

I will encourage my child to cooperate with the leaders and other youth. I understand that while attending EBC Youth, my child will be subject to the rules set by the leaders, to all reasonable extent.

I acknowledge that while EBC Youth and EBC will take reasonable steps to provide a safe environment, accidents do occur. I acknowledge that EBC will not be liable for any injury that may occur, which arises directly or indirectly from, or in connection with, EBC Youth activities.

I agree to indemnify and hold harmless EBC Youth and EBC against any and all claims, demands, suits and liability of whatever nature and howsoever arising from, or in connection with, and injury that may be suffered to my child, or that my child may cause to another person, as well as any loss or damage to property, equipment or personal effects belonging to my chold, or any other peson, arising either directly or indirectly ou of, or in connection with, EBC Youth activities.
